At New Life Counseling Services, we ensure as a part of the treatment plan development for the clients serviced at our locations that we include identifying, addressing, and problem-solving any concerns related to all the domains of our clients’ lives. When one is experiencing difficulty with addressing health, family, educational, work, and community concerns, this can negatively impact their ability to work towards achieving their treatment goals.
As master-level and doctoral level clinicians, we are trained to facilitate, coordinate, and submit referrals for services to our community partners in our region and other regions throughout the states of Kentucky. These community partner assist with addressing our client’s healthcare, employment, housing, educational, and social needs, which increases their chances of treatment success.
In coordination of these referrals, we follow up with our clients on the services they were referred for and request follows up from our community partners, when appropriate, in order to ensure their concerns are being appropriate addressed and document coordination of these services within their clinical documentation.
