If you're a new client, you will be emailed the pre-appointment forms after your first appointment is scheduled. It is important that you complete these forms prior to session so that it will allow more time for your first session, these forms typically take 15-20 minutes to complete. If the forms are not completed prior to the appointment, you will be requested to complete them in the office before you are seen.
